obviously nothing inherently wrong with this and many sexual fantasies involve power play and domination and submission - your wife doesn't have "issues" she's just curious and adventurous, which you should be grateful for whether you take her up on this or not.
There IS a risk of infection, but if that's a risk you're both willing to take it might still be worth it.

The major problem that I can think of, is that I had previously thought it to be impossible for a man to urinate whilst he is erect. This is because the nervous innervation which controls urination is "switched off" during erection so that semen comes out during sex and not urine.
However, there may be ways around this if it's a fantasy you BOTH want to fulfil.

I think you'll find it's pretty unhygienic and can cause infections. Urine's supposed to be outside the body not in and definitely not in another person.

This kind of fantasy is a bit disturbing to say the least as urinating on people during sex is a form of degredation so I think your wife has issues.
