As the other girls said, you seem to be measuring spot on, the fundal height is the height of your uterus, they measure from your pubic bone to the top of your uterus. I had this explained to me by my consultant, as since all along, my fundal height measurment has always been bigger than the weeks that I am, this, along with scans suggested that I will deliver a big baby. Since 38 weeks my fundal height has been 42cm.
Basically, the chart in your notes is based on averages across the entire population, so 1cm per week is a rough guide to the average growth on the average woman and it is (in my consultants words - or something to the affect of), "unfortunate that they don't have the resources to build an individual graph for each woman, as the size of your baby and your fundal height is down to part genetics, part ethnic origin, your diet and various other factors..."
